Chronic diseases have become a significant public health challenge worldwide, affecting millions of individuals and placing a substantial burden on healthcare systems. Conditions such as diabetes, hypertension, heart disease and chronic respiratory diseases require ongoing management and comprehensive care strategies. Nurse practitioners (NPs) have emerged as vital healthcare providers in the management of chronic diseases, bridging gaps in care and enhancing patient outcomes through their unique skill set and holistic approach to health. NPs are advanced practice registered nurses (APRNs) who have completed graduate-level education and possess the authority to diagnose and treat medical conditions. They provide a wide range of healthcare services, including health promotion, disease prevention and chronic disease management. NPs often work in primary care settings, where they play a crucial role in coordinating patient care, educating patients and families and managing complex health issues over the long term.
