One major barrier to achieving the primary goals of antiretroviral therapy is the development of drug resistance mutations to antiretroviral agents which render them less effective. For patients who are treatment experienced with many drug resistance mutations, selection of an effective antiretroviral regimen becomes challenging. We report a case of a 55 year old male on tenofovir, abacavir and nelfinavir with multiple resistance mutations to each of these agents, yet able to maintain viral suppression and high CD4 T cell counts.
