Introduction: The sending of biological materials for specific exams to the Public Health Laboratory of Amazonas (LACEN-AM) depends on documents containing essential information to guide the performance of the tests, which need to go through a quality process and in most cases the lack of data compromises the streamflow to LACEN-AM.
