Introduction

Hypothyroidism is a common endocrine disorder characterized by an underactive thyroid gland, resulting in insufficient production of thyroid hormones, primarily thyroxine (T4). This condition affects various physiological processes and can lead to a range of symptoms, including fatigue, weight gain, cold intolerance, and depression. Effective management of hypothyroidism often involves lifelong therapy with synthetic thyroxine (levothyroxine), making patient education and adherence strategies critical for optimal outcomes. Levothyroxine is a synthetic form of T4. Once administered, it is converted into its active form, Triiodothyronine (T3), in various tissues. This hormone replacement therapy restores normal levels of thyroid hormones, alleviating symptoms and reducing the risk of complications associated with untreated hypothyroidism. The initial dose of levothyroxine is often based on the patient's age, weight, and severity of hypothyroidism. It's crucial to monitor TSH levels regularly to adjust the dosage as needed. Patients are usually advised to take levothyroxine on an empty stomach, ideally 30 to 60 minutes before breakfast, to enhance absorption. While generally safe, levothyroxine can cause side effects, particularly if the dosage is too high. Patients should be informed about potential interactions with other medications, supplements, and certain foods that can affect the absorption of levothyroxine. For example, calcium and iron supplements should be taken several hours apart from thyroid medication [3].

Understanding that some patients may have anxiety or depression can aid in tailoring adherence strategies. Referrals to mental health professionals or support for managing stress may be necessary. Ongoing monitoring of TSH levels is crucial to ensure that the patient is receiving the appropriate dose of levothyroxine.
