Acute abdomen as an unusual presentation of acute myeloid leukemia: A case report

Abstract :

Introduction: Acute myeloid leukemia is a disease with abnormal proliferation of the myeloid precursors. Children with myeloid leukemia can present a wide variety of symptoms. In some instances, the patient may present with an unusual signs and symptoms. Aim: The aim of this study is to introduce a girl with acute abdomen as an unusual presentation of the acute myeloid leukemia. Case Presentation: The patient is a 15 year old girl who was brought to emergency department with acute abdominal pain, vomiting and low-grade fever. Physical examination revealed a conscious patient with temperature of 37.9�°C (orally), blood pressure of 75/55 mm Hg, generalized abdominal tenderness without rebound tenderness, and normal heart, lung, and nervous system examination. Abdominal sonography showed a hypoechoic lesion in right ovary suspicious to hematoma and also splenomegaly was detected. In abdominal CT scan, hematoma and collection around right ovary were detected. The patient received broad spectrum antibiotics and was under supervision of general surgeon for disease progression but was not operated. In laboratory data WBC: 3100/Ml and Hb: 9 mg/dL detected. So as consulted with pediatric oncologist, bone marrow aspiration was recommended. Bone marrow aspiration, immunohistochemistry and flow cytometry were in favor of acute myeloblastic leukemia with maturation (M2). The patient received chemotherapy and she was in remission with good improvement. After taking two course of chemotherapy, abdominal sonography revealed no lesions. Conclusion: In patients presenting with acute abdominal pain to emergency room, the possibility of malignancy should be considered and further evaluation may be warranted.

Mehrpour Moradi has completed his MD from Tehran Azad University of Medical Sciences, Tehran, Iran and Pediatrician degree from Shiraz University of Medical Sciences, Shiraz, Iran. He has Fellowship in Pediatric Hematology and Oncology. He has published more than three papers in reputed journals.
