Added value of using diode laser for circumcision: A retrospective study

Abstract :

Circumcision is performed routinely in our region mainly for religious and cultural reasons,sometimes for medical conditions like phimosis and balanitits.There are many techniques available for this purpose like Gomco clamping,Circumcision Ring,Bone cutting clamp or Sharp dissection method using a scissor and Bipolar diathermy. In this study,We compared the added value of using Diode Laser machine in Minimising the post procedure Pain,Bleeding,Oedema,Minimising procedure time and Achieving a better cosmesis. Key Words: Male Circumcision,Diode Laser Materials and Methods:- Circumcision were performed under local Anaesthesia for candidates below 5 months while under general Anaesthesia for babies older than 5 months. Two hundreds cases were divided into two groups :Group A had been circumcised using Gomco clamp followed by suturing skin to mucosa using rapidly absorbable sutures materials ,While Group B had been circumcised using Gomco clamp and Diode laser for cut. In both groups a tie had been applied for 24 hours . Both groups had been monitored retrospectively . Study had been conducted from February 2021 till March 2022 in Sidra Hospital in kuwait. All cases had been followed up one day,seven days and one month post procedure. Exclusion Criteria:- Babies with hypospadius or any other penile anomalies. Age Group: 6 months +/- 5 months. Results:- Retrospective close monitoring of both groups had shown the following: -Bleeding:No incidence of bleeding in group B even in older babies or babies with thicker foreskin while suturing had been mandatory in 86% of group A babies ..Tie removal was completely dry in group B while few blood drops had been seen in 76% in group A. -Pain:The post procedure need for pain killers(Non Steriodal Anti inflammatory Suppositories)was mandatory and regularly taken by group A babies while occasionally needed in group B. -Cosmesis:Better cosmetic results had been achieved in group B with proper edges coaptation and less post procedure edema. Procedure Time: Group A: 15 minutes +/- 7 minutes Group B: 7 minutes +/- 1.5 minutes Results :- Adding Diode Laser with Gomco clamp for Circumcision can decrease the procedure time and achieve better cosmetic results and can significantly minimize the possible complications like bleeding,edema or pain.
