Concomitant androgenetic alopecia and alopecia areata successfully treated with a combination of topical corticosteroids and minoxidil solution: A case report

Abstract :

Androgenetic alopecia (AGA) and alopecia areata (AA) are common forms of baldness. AGA and AA occurring concurrently in a single individual are rarely found. To the best of our knowledge, this is the first report of AGA and AA in a single individual. A case of a 23-year-old male with AGA and AA was reported. Patient presented with hair loss of the eyebrows as well as the scalp past 4 years, prior to consultation. From physical examination, there were alopecia on the frontal part of the scalp forming an M-character, on the bitemporal and occipital regions with the scalp appeared normal. Alopecia also found on both lateral regions of the eyebrows. Dermoscopy examination from alopecia in the occipital region revealed exclamation hair mark, while in the frontal regions there were hair shafts in various diameter and predominance of single hair-bearing follicles. The SALT score of 8.4% and Norwood-Hamilton scale III were established. The histopathological results from biopsy of the frontal region were found to be consistent with AGA. The patient was treated with 0.05% Clobetasol propionate cream combined with 5% Minoxidil solution. Clinical improvement was observed on day-45 marked with terminal hair growth. This case shows that both AGA and AA may coexist in a single individual and point out therapeutic success may be achieved individually as seen in this patient with conservative topical treatment and proper counseling.
