This action research was conducted aiming to develop and evaluate the nursing service system for high-risk pregnancy at Maha Sarakham Hospital and community hospitals in Maha Sarakham Province, Thailand. This study was divided into three phases: Situation analysis, development and evaluation. The instruments included a data record form for high-risk pregnancy, knowledge assessment form, a referral checklist form, a questionnaire for high-risk pregnancy care competency of nurses and a questionnaire for focus groups. Data collection was conducted from October 2012 to September 2014. The data was analyzed using descriptive statistics and content analysis. The study has found that the developed nursing service system for high-risk pregnancy was effective, matching with the identified problems and meeting the need of patients and healthcare personnel. These resulted in positive outcomes as follows: (1) The mortality rate of mothers with high-risk pregnancy was zero, (2) the newborn mortality rate decreased to 4.91:1000 live birth, (3) the complications in the mothers with high-risk pregnancy decreased to 12.9%, (4) the complications of the newborns was 10.88%, (5) the ratio of healthcare personnel with potential to take care of high-risk pregnancy cases became 93.95%, (6) the referrals from community hospitals decreased from 10.43% to 10.18%, (7) the ratio of the referring services complied with the official service plan for mothers with high-risk pregnancy increased to 98.72%, (8) the complications occurred during the referral were 0.27%, (9) the satisfaction of the service receivers was 90.39% and (10) the satisfaction of the service providers was 90.27%.
