Introduction: The goal in rhinoplasty is to achieve an aesthetically pleasing, natural appearing and normally functioning nose which stays in harmony with the rest of the face. Methods: We have looked at the records of rhinoplasty procedures performed by our senior author over a period of the 32 years (1983-2015) and noted which technique was used these included both closed and open rhinoplasty techniques. We studied the rhinoplasty cases performed in the 1960 to 1980 in the literature and concluded that the trends and teaching norms were over resection and grafting through close rhinoplasty techniques. Results: All our rhinoplasty cases performed from 1983-1987 were only by closed technique. All the cases performed from 1988-1991 were only by open techniques and since 1991 a combination of both techniques are used and currently less than 5% are performed by open (external) technique. Conclusion: We feel the open technique has greatly enhanced over rhinoplasty results and has added a new dimension to our work. We also learned to adopt the philosophy of â??preserve and conserveâ? use suturing techniques wherever possible rather than excision and grafting to achieve a finesse in rhinoplasty.
