Much of the care that is provided to individuals with chronic illness occurs in the community setting. With the increase in rates of chronic diseases and the growth of the elderly population, it is imperative that registered nurses understand how to provide care and promote health in the community setting. Innovative University-Practice Setting partnerships facilitate student learning and also promote health and provide care for populations that are underserved. This poster will present The Care-Link Model of care and provide examples of several innovative university-practice partnerships and strategies to put them in place. These partnerships increase student clinical competency in the community setting, save public resources and increase care to elders and the disabled, vulnerable populations who are underserved.
