In the treatment of rectal carcinoma, if it is operable, the management consists of local wide excision or radical dissection with permanent colostomy with adjuvant chemo or radiotherapy according to the disease stage. Radical approaches like Anterior Resection and Abdominoperineal resection (APR) are more used in most of the set ups. In recent era of minimal invasive surgeries, laparoscopic abdomino-perineal resection approach is being used quite frequently. The need for large abdominal incision can be overcome by using a 4-port laparoscopic approach. In this case of rectal carcinoma stage 1 we have used a minimal invasive laparoscopic approach with 4 abdominal ports and a single perineal incision to achieve good sigmoid colon and rectal dissection along with perineal dissection upto levator ani and resection of rectal mass. Post-operative results were good with the least possible hospital stay of the patient.
