A phenomenon known as hidden penis is a condition in which the penis is partially or completely hidden below the surface of the skin. Most patients have normal corporal length but function and aesthetic appearance are hindered. Penile shortening and erectile dysfunction has been reported as a complication of radical prostatectomy, as well as related to androgen deprivation and radiation therapy. Penile dysmorphic disorder can lead to depression, low self-esteem and further negatives effects on the male libido and erectile function, thus creating a negative spiral. Correction of buried penis presents a unique surgical challenge, yet its management may result in improved penile visual and functional length. Commonly penile implants are placed separately from concomitant penile procedures due to fear of infection. We present a case report on a 66-year-old patient with history of prostate cancer treated with open radical retropubic prostatectomy and salvage external beam radiation therapy who underwent a buried penis correction with an immediate implant placement, which has not been previously described. In presenting this case, we demonstrate a one-stage approach of penile implant placement and concomitant buried penis correction, which leads to an improvement in both sexual function and aesthetics. One year post-operatively, our patient has a Sexual Health Inventory for Men (SHIM) score of 25/25. The patientâ??s penile length when flaccid and outstretched increased from 3.5 to 6 inches. He is very satisfied with his cosmetic outcome and is able to have penetrative sexual intercourse, which he began at two months postoperatively.
