Developmental cysts are the most common congenital entity encountered in the peri-rectal space and include epidermoid cysts, dermoid cysts and enteric cysts. We encountered a case of perirectal epidermal cyst in a 48-year-old woman. The patient presented with chronic history of pelvic pain and constipation. Patient gave a history of the pelvic cystic mass for 23 years that was increasing in size with symptoms. Magnetic resonance imaging revealed a cystic lesion (11.6Ã?Â?Ã?Â?8.2 cm) has a mass effect on rectum, which is pushed laterally and is stretched. Histologically, the cyst was diagnosed to be an epidermoid cyst. Surgically, the cyst was removed with hysterectomy for large fibroid with smooth post-operative recovery. We are illustrating comprehensive review of the literature for this rare case.
