Pharmacotherapy as a tool for the prevention of perioperative complications

Abstract :

State of The Problem: Perioperative period is related to risk of complications and even death, especially for patients with chronic comorbidities. 30-day mortality in patients undergoing non-cardiac surgery remains 1.3–1.9%, myocardial infarction–up to 6.3% and also there is the risk of cerebrovascular, pulmonary complications, metabolic derangement, impaired glycemic control, infection, iatrogenic complications. Most of these complications are preventable with the use of rational perioperative pharmacotherapy but there is not a common view among the clinics about the perioperative pharmacotherapy approaches. Principles and Current Evidence of The Perioperative Pharmacotherapy: A complete medication history should be obtained and multidisciplinary team should review this history. Medication history should include prescription medications, herbal/ complements and information about drug abuse. According to patient and surgery specifics multidisciplinary team, anesthesiologist, surgeon, clinical pharmacologists and consultants should concern about the analgesics, antithrombic agents, antibiotics, gastric protection, fluid and blood components infusion, glycemic control, usage of vasoactive and cardio protective medications, glycemic and metabolic control, liver and kidney functions, mental problems and other complications which can be seen during perioperative period. Multidisciplinary team should achieve consensus in all cases and situations until patient discharge from the hospital. This team is responsible for stop and restart or replace of medications during perioperative period. In situation, when unique approaches are not created for all specific cases, it is very important the relations between clinics and exchange the experience for achieving of better results. Future clinical trials are necessary for optimization of perioperative pharmacotherapy which is a great tool in hands of clinicians for the improving of surgical outcomes.

Biography :

Luiza Gabunia has graduated PhD in 1994 Pharmacology Institute in Moscow. She is an Associate Professor of the Medical Pharmacology Department, Director of the Scientific Research- Skills Centre of Tbilisi State Medical University. She has published more the 100 scientific papers in local and international journals. She is the author of 10 books and 5 monographs. The main direction of her professional activity as a pharmacologist, in addition to academic and scientific activities, is the fight against polypharmacy and the implementation of rational pharmacotherapy in the medical practice of Georgia, which is served by the documents, manuals and postgraduate training programs, issued by her. She is the organizer or speaker of up to 40 international or local scientific conferences, as well as a trainer of various training courses in the direction of teaching scientific research skills, as well as in clinical pharmacology and rational pharmacotherapy. She is the President of the Biomedical Research Development Association, a Member of the Georgian Association of Pharmacologists, member of the European Association of Clinical Pharmacology and Therapy and the International Council of Medical Sciences (CIOM); Founder of the Association Health and Women.
