FUE hair transplant has evolved to be the most common and predictable technique of surgical hair restoration in last two decades with minimal down time, absence of linear scar and with time tested results. Since the introduction of FUE technique, there has been an array of improvement in the technique of local anesthesia administration, graft extraction techniques, the surgical armamentarium and the implantation techniques. We herein discuss about the recent advances in the graft extraction techniques for a better, faster and predictable graft yield and about the different types of punches, machines and our experiences associated with it. Accurate graft implantation is also a critical step in hair restoration surgery. As the hair transplantation has evolved, the size of the graft and recipient micro-incisions has decreased while the number of graft and placement density has increased. Implanting large number of micro graft into this micro incision is a technically difficult maneuver which involves a significant risk of trauma to the grafts. We also discuss the graft survival rate, graft density, the caliber of the graft, the operating time required and the operator�s comfort between two different implanting techniques, manual and with readymade implanters.
