Robotic surgery is a promising feather in the cap of minimally-invasive procedures. Since the advent and approval of the da Vinci robotic system, radical onco-surgeries have become revolutionized. One such procedure is robotic nephronsparing surgery done for localized renal cancer patients. Robotic surgery has heralded newer challenges for the attending anesthesiologist, especially in patients with serious co-morbid conditions, like hypertrophic cardiomyopathy. This is a serious, genetic cardiac condition in which there is dynamic left ventricular outflow tract obstruction, sometimes leading to arrhythmias, cardiac failure or sudden cardiac death. The anesthetic challenges were magnified with the concurrent presence of difficult airway in the patient due to past history of radiotherapy to the neck for carcinoma of the larynx. The major anesthetic concerns and the goals of management are discussed.
Uma Hariharan, is an Assistant Professor of Anaesthesiology and Intensive Care, at Dr Ram Manohar Lohia Hospital & PGIMER, New Delhi, which is one of the premier teaching medical institutes of India. She has a keen interest in cancer, following her fellowship in onco-anesthesia from Rajiv Gandhi Cancer Institute and Research Center, New Delhi. She has more than 75 publications to her credit in various indexed national and international journals as well as several book chapters. She is also in the Editorial Board of several reputed journals and continuing to make fruitful contributions. She is also trained in Hospital Management (PGDHM), Transplant anesthesia, Ultrasound-guided regional, and Palliative Care/Medicine.
