Aim: To review the safety process of HAMs in chemotherapy intrathecal procedure room and to improve compliance of
independent double check practice (performance) before administration of intrathecal chemotherapies and documentation).
Significance of the Problem: Safety of high alert medication is an international patient safety goal. Independent double check
against five rights of high alert medicine before administration and its documentation. Safety of HAM is a patientsā?? safety
concern, impacting patientā??s health. Joint commission standards addressed High Alert Medicationsā?? (HAMs) issue in Medication
Management and uses (MMU) and elaborated that HAMs related error causes sentinel event, prolong hospital stays, increase
suffering and cost burden for patients/families as well as organization.
Methodology: Three months retrospect charts of 68 patients who received IT reviewed (43 Adult and 25 Pediatrics) pre and post
implementation. Independent double check compliance Pediatric was 44% and adult independent double check compliance was
84%.
Results: After implementation the post intervention audit results of pediatric patient chart were 100% compliant and adult
patients chart review was 93% compliant. Improvement will be followed to achieve bench mark of 100% compliance for adult
patients as well.
Conclusion: Independent double check is a paramount safety ensuring process and must be carried out before administration
of high alert medication as recommended by Institute for Safe Medication Practices (ISMP) At our hospital, strategies of ISMP
are implemented and monitored across the hospital where HAMsā?? are handled. In particular, chemo bay till the gains to achieve
100% compliance for chemo adult patients as well. An improvement project followed by Plan, Do Check, Act (PDCA) cycle for
chemo pediatric patients is continuously monitored for sustainability.
