Partial or complete loss of N-A complex is common after reduction mammoplasty in severe cases of macromastia. 16 consecutive patients complaining from macro-mastia were included in this study. Preoperative spiral CT angiograms to trace the blood supply to N-A complex were done to those patients. Custom made pedicle flaps according to the results of the angiograms with the help of handheld Doppler 10 mhz. were done. 9 supero-medial flaps, 4 lateral, and 3 inferior flaps for reduction mammoplasty. All cases showed good results as regards survival and sensation of nipple-areola complex. Cases were followed up for 6 months. Results showed that spiral CT angio is a helpful tool for planning of the glandular flap for reduction mammoplasty in sever macro-mastia.
