Total anomalous pulmonary venous return is a rare congenital heart disease in adult and usually requires
surgical correction within the first few months of life. We report results of a 54 year old man with total
supra-cardiac anomalous pulmonary venous return who was presented with congestive right heart failure
and underwent completed surgical correction.

Introduction: The abnormal pulmonary venous return (APVR) is a rare disease, with a frequency of 0.7-1.5%
among congenital heart diseases [6]. Most patients with APVR had a 50% mortality rate in the first 3 months
after birth and lived for an average of 2 months, most infants died of acute heart failure without surgical
intervention [3]. Therefore, the case of male patients with APVR on the heart who have never undergone
surgery has survived to age 54 as a rare case. We successfully repaired the derivation of the total pulmonary
venous return into the left atrium when the patient's symptoms became progressively worse after 4 years.
