Abstract :

The trichoscope is an indispensible, noninvasive tool in the diagnosis of hair and scalp disorders. It not only highlights subtle tricoscopic points invisible to the naked eye but also serves as prognostic and monitoring tool in therapeutic management. We studied trichoscopic findings in 200 patients. Trichoscopy was performed with non-polarized Heine delta 20 mini dermoscope (10Ã? magnification) using liquid paraffin as contact medium. 116 males and 84 females were included. There were 41 patients with androgenetic alopecia , 39 with alopecia areata , 25 with telogen effluvium , 21 with FPHL , 15 with tinea capitis , 13 each with lichen plan0-pilaris and seborrheic dermatitis , 11 with scalp psoriasis , 10 with DLE etc. Representative findings were as follows - Androgenetic alopecia showed yellow dots honeycomb pigment , peripilar sign etc. Alopecia areata had black dots , tulip hair , pigtail regrowing hair etc. Telogen effluvium showed short regrowing hair , peripilar erythema , yellow dots etc. Tinea capitis had comma hair , corkscrew , zigzag hair etc, Lichen plano-pilaris showed white dots , blue grey dots in targetoid pattern , peripilar cast etc. DLE had arborizing red loops , follicular plugging , white areas etc. Dimeter diversity was less than 20%. Trichoscopy is an excellent diagnostic tool. Trichoscopy helped to reach conclusive diagnosis in patients in whom diagnosis was not clear. We conclude that trichoscopy is a relevant investigation in patients with alopecia and has a definite role in the diagnosis of difficult cases.
