H yperuricemia is common in patients with chronic kidney disease. Despite recent studies showing a potential benefit of uric acid lowering therapy in delaying the progression of chronic kidney disease, its role in the treatment of patients with chronic kidney disease remains controversial. Unlike acute uric acid nephropathy (urate nephropathy), the mechanism of renal injury from chronic asymptomatic hyperuricemia is poorly understood. Animal data has shown that hyperuricemia is associated with glomerular hypertension and interstitial fibrosis. However, human studies have not definitively shown clinical benefit. Uric acid lowering agents can also have significant side effects in patients with chronic kidney disease due to impaired renal clearance. Based on the current body of evidence, the use of uric acid lowering therapy to delay the progression of chronic kidney disease is not indicated
